@Fizzbinn kindly mailed me the print he made of this bezel, the results are pretty good! Here are a few shots of the jaz drive with it's original box I got it in. The disk was unopened so I don't believe this drive was ever used, the SCSI cable was still neatly factory wrapped as well.

Thanks again to @phunguss and @Fizzbinn! Hopefully this helps someone else out in the future!
