So when you fully boot into safe mode, what is the resolution? Do you have any other boot drives or optical media to boot from? Sounds like a software conflict.

Could it be trying to go to a resolution that is not supported? What resolution is your CRT? Try connecting your CRT, fully boot, set the resolution as low as you can go and shutdown, remove CRT, add ADC monitor and boot back up. I don't have a 20" to test things.. but I do have 17", 22", and...

Here is one version of an adapter pinout. I see that your KVM can output to Apple DB15 or VGA DB15, so it must have internal intricacies/adapter. I would agree with @joevt that the cables required are likely straight through from the 3 computers to the KVM.

The video shows you are still getting the 4 beeps. Try disconnecting the hard drive and only leave the cdrom attached. When you power up, hold down Command-Option-R-P and see if you get other sounds (cycle through 3 boot chimes to fully reset everything).
