I came here because I had a similar issue with a v2 between an SE/30 (works) and a performa 637 and 6300 (didn't work). It turns out that one of my image numbers conflicted with the performa's CD-ROM. Switching to a different number (from 3 to 4) solved my issue and now it works on all machines.
