BlueSCSI v2 disk read errors

I recently bought a brand new BlueSCSI v2 for use in my Power Mac G3 Blue and White and am having issues.

I keep getting disk read errors on my BlueSCSI despite purchasing a brand new Sandisk Extreme Pro high performance SD card that is 200MB/S read speed / 90MB/S write speed so we know the SD card is not the issue.

I do have the SCSI bus terminated with the built in terminator on the SCSI cable in my Mac so we know termination should not be the issue.
I had to turn off the termination jumper on my BlueSCSI in order for my Mac to see the BlueSCSI

The BlueSCSI is the only drive on the SCSI bus as I have unplugged the 9TB IBM Scsi hard drive.

I am using an adapter JXSZ SCSI HD 68 Pin to IDC 50 Pin Adapter Female-Female in order to use the BlueSCSI with my G3 along with the stock Adaptec SCSI card and SCSI cable that come stock with the G3 Blue and White from the factory.

I know the SCSI card and cable are good as they work perfectly with the IBM SCSI hard drive that i now have unplugged for the BlueSCSI to be the only SCSI drive.

I have tried both the stock IDE CD-ROM drive and the emulated CD drive on the BlueSCSI and both give disk read errors when trying to write to the hard drive image when trying to install Mac OS 9.2.1

I have tried creating a new hard drive image using Disk Jockey and still same errors.

The BlueSCSI toolbox seems unable to detect the BlueSCSI

My Mac G3 BW is seeing the BlueSCSI and booting from the emulated CD drive on the BlueSCSI but writing files seems to be an issue.

If anyone can help me out that would be appreciared.
