    Thanks for the suggestions to help narrow this down. Unfortunately I don't have an external SCSI hard drive. I'll need to create a floppy boot disk and try it to see if that works. I'll also try to boot from different OS version to see if that helps with anything.

    Fixed! Full recap resolved the issue! It's kinda odd because the board has been already recapped once, but perhaps a long time ago. I think they also missed a couple caps. I did some additional reflows, so that also could've helped. Along the way I also noticed the ROM on the logic board...

    Highly recommend against this. I tried this out initially, this will cause the water to heat unevenly and anything with-in close proximity of the heater will get bleached stains. Buy a cheap used sous vide instead.
