Earliest I can find on Wayback Machine is 1998, products for early PPC machines. So my guess would be a TV tuner card for Pre-G3 machines, like 7100/8100?
Look at the back of this 6500, looks like a tuner card similar, though I am unfamiliar with the slot connector internally.

I had no idea that was a thing. I wonder if it accommodated some special/regional analog TV standard that Apple did not with their official cards?
